About the Role :

The Admission Coordinator (AC) is responsible for facilitating the admission process for patients into hospice care. They will utilize their clinical expertise, compassion, and strong communication skills to assess patients for hospice eligibility, facilitate their transition into hospice services, and provide support to patients and families during a challenging time.

Role Requirements :

• RN with active RN licensure in the state of employment.
• Associate or bachelor’s degree in nursing from an accredited program.
• Two (2) years of nursing experience in hospice, home health, emergency room (ER), intensive care unit (ICU), or other related experience.
• Must have reliable transportation, proof of current automobile insurance, and a valid driver’s license.
