HSS is a private, non-profit community resource agency founded in 1973, providing confidential COA-accredited services.

Human Support Services is seeking to add a full-time Case Manager, to our premier team of mental health professionals to provide mental health treatment in Monroe County, IL. This position works with our supported residential housing clients / population.

ILLUSTRATIVE EXAMPLES OF POSITION DUTIES :

• Case Management
• Advocacy, referral and linkage to Community Support Services
• Community Education
• Using community and resource knowledge to engage clients
• Documentation of clinical services
• Meet agency standards for billable services
• Utilize a variety of therapeutic interventions to address clinical needs.
• Complete required data entry and paperwork in accordance with program and agency policies and procedures, including but not limited to opening requirements, assessments, treatment / recovery plans, progress notes, treatment authorizations and authorization of medical necessity.
