Substance Use Case Manager

Concern Housing is seeking a Substance Use Case Manager who will be responsible for providing case management and chemical dependency counseling services directly to clients receiving Mobile Residential Support Team services. The Substance Use Case Manager is considered a resource in that they should be familiar with community resources and services to which they may help their clients gain access and also be an advocate for their clients.

Position : Substance Use Case Manager

Schedule : Monday - Friday; 9am - 5pm

Location : Middle Island, NY

Salary : $57,500

Responsibilities Include but are not limited to :

• Provision of chemical dependency counseling services to assist individuals to develop the skills needed to prevent

relapse, promote recovery and independence and increase self sufficiency. Counseling shall include assessment of

needs, identifying triggers, increasing coping skills, development of resources, service planning, referral to necessary

treatment providers, case coordination and collaboration, community integration, supportive counseling and crisis

intervention.

Requirements : CASAC or CASAC-T credential is required for this position. Bachelor’s Degree in Psychology, Rehabilitation, Social Work or other related discipline, plus 2 years’ experience working in a similar setting and / or working with people in recovery from chemical dependence. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care
