- Be part of a team dedicated to supporting each other in daily operations and challenges of community pharmacy.
- Improve patient outcomes through medication reconciliation at hospital admission and discharge.
- Staff in the outpatient pharmacy and collaborate with interdisciplinary care teams to fulfill hospital discharge orders.
- Balance the needs of community patients served.
- Staff rotating Medication Reconciliation Pharmacist shifts with other outpatient pharmacy staff.
- Be adaptable and ready to collaborate with the pharmacy team and all area healthcare providers.
Qualifications :
Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD Pharmacy degree.Current license to practice pharmacy in Colorado or ability to obtain within 60 days of hire.Immunization Certified with an active BLS card.Able to work independently with minimal direct supervision.Able to handle frequent interruptions and adapt to changes in workload and work schedule.Ability to set priorities, make critical decisions, and respond quickly to emergency requests.Experience in meeting the pharmaceutical needs of various patient demographics including neonatal, pediatric, adolescent, adult, geriatric, psychiatric, oncologic, obstetric, chemically dependent, and rehabilitation patients.Community pharmacist experience preferred.Why is This a Great Opportunity :
