  • Be part of a team dedicated to supporting each other in daily operations and challenges of community pharmacy.
  • Improve patient outcomes through medication reconciliation at hospital admission and discharge.
  • Staff in the outpatient pharmacy and collaborate with interdisciplinary care teams to fulfill hospital discharge orders.
  • Balance the needs of community patients served.
  • Staff rotating Medication Reconciliation Pharmacist shifts with other outpatient pharmacy staff.
  • Be adaptable and ready to collaborate with the pharmacy team and all area healthcare providers.

Qualifications :

  • Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD Pharmacy degree.
  • Current license to practice pharmacy in Colorado or ability to obtain within 60 days of hire.
  • Immunization Certified with an active BLS card.
  • Able to work independently with minimal direct supervision.
  • Able to handle frequent interruptions and adapt to changes in workload and work schedule.
  • Ability to set priorities, make critical decisions, and respond quickly to emergency requests.
  • Experience in meeting the pharmaceutical needs of various patient demographics including neonatal, pediatric, adolescent, adult, geriatric, psychiatric, oncologic, obstetric, chemically dependent, and rehabilitation patients.
  • Community pharmacist experience preferred.
