Job Summary :
The LPN performs under the direct supervision of a Registered Nurse and is delegated an assignment by him / her to achieve quality patient care. The LPN shall assist the RN in the implementation of the nursing process by providing direct patient care as evidenced, but not limited to, patient care documentation, direct observation, and patient evaluations. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to read data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his / her age specific needs, and to provide the care needed. Completes the Care Plan Progress Note each shift. Performs Medication Reconciliation Process on Admission, discharge and as needed. Performs required documentation each shift. Demonstrate knowledge and skills to perform wound care with dressing changes, demonstrates knowledge and skills to perform blood glucose monitoring and insulin administration.

Qualifications : Education / Skills
Graduate of a recognized non-online LPN program required.
Work Experience :
Work experience preferred.
Certification / Licensure-DUE UPON HIRE
