Job Summary:
The Registered Nurse Case Manager will provide skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Responsibilities include patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. Strong communication and collaboration with patients, providers, and team members are essential.

Responsibilities:
- Provide skilled nursing care, case management, and hospice care.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Communicate effectively with patients, providers, and team members.
Required Skills & Certifications:
- Clinical Assessment, Medication Administration, and Treatment Planning skills
- Minimum of 2 years of hospital, home health, or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelor's degree in Nursing or related field
- Driver's license, car insurance, and automobile for field work
Preferred Skills & Certifications:
- Specialities in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room
Special Considerations:
- Must be willing to float between like units based on need.
- Must be willing to work 2 weekends every 6 weeks.
- All requests for time off (RTO) must be submitted at the time of application.
