CHC is seeking a licensed Family Nurse Practitioners to join our team! This can be either part-time or full-time . The Advance Practice Practitioner - Family joins a collegial environment that promotes exceptional care for patients, teamwork, quality improvement and innovation. This role includes possible evening and weekend shifts.
Basic Qualifications
Education
- Graduation from program of study accredited by the appropriate professional board
- Board certification or eligibility for board certification.
Licensure for practice in Vermont.
Completion of requirements for successful CHC credentialing.
Ability to work effectively with a variety of ethnic, political, and socioeconomic groups and individuals.
Ability to work independently as appropriate to level of training and experience, and to solve problems creatively and with initiative.
Willingness to work flexible hours and flexible locations.
Commitment to supporting the mission and principles of the Community Health Centers.
Must be CPR certified.

Board certified or eligible for board certificationFor Physician Assistants, must have CAQ in PsychiatryCPR certificationLicensure for practice in the State of VermontCompletion of requirements for CHC practitioner credentialingCompensation commensurate with direct experience within the range of $109,946- $130,394 base pay annually.
