Job Title : Registered Nurse Job Specialty : Case Manager / Discharge Planner Job Duration : 13 weeks Shift : Day, 5x8-Hour (08 : 00 - 16 : 30) with rotating weekends Guaranteed Hours : Experience : Minimum 2 years required, must have 1 prior travel assignment License : Must have a valid DC License in hand (for DC locations) Certifications : BLS, RN, or LMSW / LGSW / LCSW Must-Have : Experience with discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters Job Description : - Serve as a Social Worker / Discharge Planner- Work 40 hours a week during the day shift from 8 : 00 a.m. to 4 : 30 p.m.- Be available to work every 5-6 weekends for one shift as needed- Possess skills in Cerner, Acute Hospital, Insurance Company, and Long Term Acute Care / Rehab / Skilled Nursing- Preferred experience in Home Health, Hospice, telephonic, and previous charge experience- Demonstrate skills in Case Management / Utilization Review including Admission Criteria, Benefits Eligibility, Care Coordination, Discharge Planning, Needs Assessment / Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, and Workers Comp Case Management- Possess required regulatory skills in CMS (Centers for Medicare and Medicaid Services), Department of Health, HIPAA (The Health Insurance Portability and Accountability Act), and The Joint Commission / Core Measure / National Safety Goals- Preferred regulatory skills in Workers Compensation- Manage patient diagnoses including Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, and MICU- Adhere to a business casual dress code ensuring a clean, neat, and professional appearance (no scrubs)- Mandatory flu vaccine compliance with no exemptions allowed- Complete orientation of 28 hours at 50% non-billablePlease note that the position requires a commitment to not request time off during the first two weeks of the assignment.
