Job descriptionConduct in-home nursing assessments (physical, psychosocial, environmental) Develop and implement individualized care plans Provide skilled nursing care : wound care, medication management, infusion therapy, catheter care, palliative care Monitor patient condition and report changes promptly Educate patients and caregivers on disease management and self-care Coordinate care with interdisciplinary team members Maintain accurate documentation in EHR systems Follow infection control and safety standards Participate in case conferences and quality improvement activities Active Registered Nurse (RN) license in the state of employment 2-3 years of nursing experience preferred (home health experience a plus) Strong clinical assessment and critical-thinking skills Excellent communication and patient education skills Knowledge of home health regulations and standards Ability to work independently and manage caseloads EHR proficiency Wound care experience preferred Valid driver's license and reliable transportation
