We're seeking an experienced Hospice Registered Nurse (RN) who believes that care goes beyond clinical skill - it's about compassion, connection, and preserving dignity at every stage of life. What You'll Do

  • Deliver skilled, compassionate hospice care according to each patient's individualized plan.
  • Conduct thorough assessments and collaborate with the interdisciplinary group (IDG) to build and adjust care plans.
  • Educate patients and families on the end-of-life process, comfort measures, and available support services.
  • Monitor patient conditions, report changes promptly, and coordinate adjustments with the attending physician.
  • Supervise Hospice Aides and LPNs to ensure consistent, high-quality care.
  • Maintain accurate documentation in compliance with all state, federal, and accreditation standards.
  • Participate in IDG meetings, quality initiatives, and staff development opportunities.
  • Provide insight and emotional support to families, offering calm and guidance when it's needed most.
  • Support patients holistically - medically, emotionally, and spiritually - to enhance comfort and quality of life.

What You'll Bring

  • Graduate of an accredited nursing program.
  • Current Registered Nurse (RN) license in Illinois. Minimum of 1 year of RN experience ; hospice experience preferred.
  • Excellent clinical judgment, assessment, and problem-solving skills.
  • Strong communication and organizational abilities.
  • Valid driver's license, reliable transportation, and current insurance.
