Job descriptionProvide primary care rounding in SNF and ALF settings Diagnose and manage acute and chronic medical conditions Develop and implement individualized care plans Conduct clinical case reviews and share best practices Foster strong relationships with patients, families, and facility staff Estimated annual compensation : $210,579 $271,579 (no cap on productivity income) Comprehensive benefits package Paid professional liability insurance with tail coverage Access to professional development tools, CME, and leadership training Clinician wellness and referral programs Career advancement opportunities Board-certified or board-eligible physician Experience in post-acute care preferred but not required Active and unrestricted Delaware medical license (or in process) DEA license with prescriptive authority Located in New Castle County along the I-95 corridor between New York City and Washington, D.C. Position Highlights :
- Full-time position (5 days / week) providing primary care in 2 skilled nursing facilities and 1 assisted living facility
- Flexible rounding schedule to create work / life balance

- Collaborate with interdisciplinary teams, facility staff, and leadership
- Participate in care coordination initiatives and quality improvement
