Requirements  Annual hospital deliveries : 120

Anticipated OBHG deliveries – Unassigned

patients vs Private patients : 10 / month

Expected OBHG patients encounters

per day : Office : 15 Hospital : 5

Current LampD coverage model : Inpatient hospitalist transitioning to MHAS

NICU level : 1

Anesthesia coverage model : On call at night

Clinic / outpatient

responsibilities if any : Outpatient

clinic included

Who will provide back up to OBHG in

the event of multiple emergencies OB / Gyn Call Schedule? : Community provider and hospital physicians ED

general surgeons

Gyn responsibilities : From clinic and ED call

Hospitalists Required to Perform

Circumcisions No

Will OBHG back-up private or

hospital employed Certified Nurse Midwives or Family Practice Physicians : Yes- CNM

MFM coverage : n / a

Is Board Eligible and within 5

years of residency accepted yes

Schedule  Transitioning to MHAS model
