Haven Hospice Tucson is seeking a compassionate and skilled Hospice Nurse Practitioner to join our team. This position focuses on face-to-face patient visits, offering flexibility and the opportunity to make a meaningful impact on patients and their families.

Pay : $150 $175 per visit

Responsibilities :

• Conduct face-to-face hospice visits with patients in their homes.
• Assess, monitor, and document patient conditions in compliance with hospice regulations.
• Collaborate with interdisciplinary team members to develop and implement care plans.
• Provide education and support to patients and their families regarding disease management and end-of-life care.
• Ensure timely and accurate documentation for billing and regulatory purposes.
