Location / Schedule : 168 Old Brent Road, Forsyth, Ga Monroe County. Monday-Friday 8 : 00am-4 : 00pm.

Key Responsibilities :

• Liaison between the Support Coordinators, Case Managers, and families.
• Monitors : Activities in day center; bowel tracking & changing schedules; medications; credentials; individual documentation books; and training compliance.
• Creates and follows the monthly schedule; coordinates staff meetings.
