General Statement of Job
The role of the Case Manager for the Reentry Program is to help Justice Involved adults coming out of incarceration to make successful transitions back into the community. The Case Manager performs criminogenic risk assessments and intakes with clients, based upon referral sources. He or she will be adept at performing risk-based case management services to ascertain the client’s degree of vulnerability and the probability of reoffending. This position is funded by the NC Department of Public Safety (DPS) and will report to the Lead Case Manager. Administratively, it falls in the Division of Community Outreach and Engagement at Pitt Community College, who functions as the Intermediary Agency to the Local Reentry Council (LRC). The Lead Reentry Case Manager supervises this position.
Essential Job Functions
Manages referrals for service from a Pitt County Probation and Parole and a other sources.
Works with and leads an interdisciplinary team to assist clients with obtaining identification, employment applications, resume and cover letter development, interview preparation, job search and retention skills, and budgeting and financial planning
Respond to general inquiries and assist in resolving client crises, as necessary
In conjunction with the Lead Case Manager coordinates walk-in services at the front desk and helps the Lead Case Manager with volunteer and internship assignments
Makes client referrals for service to community based organizations
Builds and maintain community partnerships
Has the ability to work independently in a mobile environment
Attends organizational and community-based meetings and events as an ambassador of for the program
Engages in all training required by DPS Reentry and Pitt Community College, related to the job
Facilitates pre-release Planning class at various agencies where Justice Involved Individuals are (prisons, jails, detention centers, etc.)
Provides direct assistance to clients for employment related needs
Facilitates training and education opportunities for clients by working closely with the Pitt Community College Outreach Division
Works with the Lead Case Manager and the HRD Department at Pitt Community College to properly place clients into LIFE SKILLS classes as needed
Links and schedules appointments with community provider in an attempt to develop client- based resources
Reports out as directed to the LRC and the Executive Committee with reports and data on a monthly basis
Is adept at the utilization of various forms of social media that can be used to enhance the program’s status and outreach to the wider community
Inputs/tracks all participant data and services provided by utilizing case management and data collection system
Develops and maintain a list of employers who are willing to hire justice-involved individuals.
Assists participants in making a successful transition back in the community by providing service coordination, support and guidance.
Assists participants in creating a plan of action, monitor their progress, along with follow up until completion of the plan.
Links and schedule appointments with community providers.
Cultivates and educate a pool of potential employability training, job seeking resources.
Provides short-term counseling and works with clients to enhance their self-esteem and self- confidence through Motivational Interviewing and a strengths-based approach.
Maintains case notes, files, and client database as specified by the Program Manager/Coordinator and/or the Sr. Case Manager and/or the Department Coordinator.
Refers agency applicants, participants, graduates, and community residents to programs (e.g. housing services, long-term counseling, child-care, substance use counseling, mental health counseling, social services and legal services) that can assist them to resolve issues preventing them from participating in the labor market or adjusting to their return home.
Provide follow-up supportive services according to established guidelines.
Performs all other duties related to the job.
Personal Characteristics:
- Knowledge of personal computers including Microsoft Word, Excel and Internet
- Reliable transportation, valid driver’s license and insurance
- Ability to work some evenings and weekends

- Ability to work with minimal supervision as well as a part of the team
- Ability to effectively and directly work with diverse populations
- Comfort working within secure jail/prison facilities with incarcerated persons
- Able to make decisions based on experience, fact and sound reasoning
- Must meet background and security clearance guidelines for facility access
Hours of Service: This staff member shall be responsible for providing the above prescribed duties for an average of 40 hours per week.

Equipment: Computer, fax, scanner, printer, copy machine, and multi-line phone
Language skills: Must be able to speak, read and write in English
License Requirement: Required to have a valid driver’s license and properly registered and insured motor vehicle that meets the minimum level of coverage as defined under the North Carolina Department of Motor Vehicle Code or able to obtain such within three months of employment.
Qualifications or Standards
A Bachelor’s degree is required, in the area of Social Work, Criminal Justice, Psychology, Human Services or a related field. The degree must be from a College or University, accredited by one of the six accreditation bodies in the United States recognized by the Department of Education (DOE). A minimum of three years of verifiable case management experience is required. Individuals who have worked with or provided services for Justice Involved Individuals are preferred.

Includes exercising discretion in determining data classification, and in referencing such analysis to established standards for the purpose of recognizing actual or probable interactive effects and relationships.
Human Interaction: Requires the ability to provide guidance, assistance, and/or interpretation to others regarding the application of procedures and standards to specific situations.
Equipment, Machinery, Tools, and Materials Utilization: Requires the ability to operate and control the actions of equipment, machinery, tools and/or materials requiring complex and rapid adjustments.
Verbal Aptitude: Requires the ability to utilize a wide variety of reference, descriptive, and/or advisory data and information.
Mathematical Aptitude: Requires the ability to perform addition, subtraction, multiplication, and division; the ability to calculate decimals and percentages; the ability to utilize principles of fractions; and the ability to interpret graphs.
Functional Reasoning: Requires the ability to carry out instructions furnished in written, oral, or diagrammatic form. Involves semi-routine standardized work with some latitude for independent judgment concerning choices of action.

Physical Ability: Tasks require the ability to exert light physical effort in sedentary to light work, but which may involve some lifting, carrying, pushing and/or pulling of objects and materials of light weight (5-10 pounds). Tasks may involve extended periods of time at a keyboard or work station.
Dexterity: Requires the ability to perform semi-skilled coordinated movements, such as those required to operate office equipment.
Sensory Requirements: Some tasks require the ability to perceive and discriminate visual cues or signals. Some tasks require the ability to communicate orally.
Environmental Factors: Essential functions are regularly performed without exposure to adverse environmental conditions.
