Job Title : LPN – LTC
Location : Oswego, NY
Shift : 06 : 00 AM - 02 : 30 PM
Guaranteed Hours : 40 Hours
Duration : 13 Weeks / Contract
Key Responsibilities :
Provide nursing care to residents in accordance with their care plans and individual needs.
Administer prescribed medications and treatments, and monitor residents for side effects or complications.
Perform routine nursing assessments, document patient progress, and update care plans as necessary.
Assist with daily activities of living (ADLs) including bathing, dressing, feeding, and mobility support.
Monitor and record vital signs, and report any changes in condition to the supervising RN or physician.
Collaborate with the interdisciplinary care team (including RNs, physicians, social workers, and other healthcare professionals) to provide holistic care for each resident.
Required Qualifications :
Licensed Practical Nurse (LPN) license in New York State (NYS).
Active BLS Certificate.
Minimum of 2 years of experience in Long Term Care (LTC) or related healthcare field.
Ability to float between different units of the facility as required.
