Position Summary :

Revere Health is seeking a dedicated and compassionate Advanced Practice Practitioner (APP)either a Nurse Practitioner (NP) or Physician Assistant (PA)to join our dynamic Cardiology team. This role supports our mission to deliver high-quality, value-based care to patients with cardiovascular conditions.

Key Responsibilities :

• Conduct comprehensive patient assessments, including history, physical exams, and diagnostic evaluations.
• Collaborate with cardiologists to develop and implement individualized treatment plans.
• Manage chronic cardiovascular conditions and provide follow-up care.
• Educate patients and families on disease prevention, treatment plans, and lifestyle modifications.
• Participate in quality improvement initiatives and interdisciplinary team meetings.
• Document patient encounters accurately and timely in the EMR system.
