Responsibilities

Terros Health is hiring a Full-Time HIV Outreach Specialist for our Indian School location in Phoenix, AZ.

High School Diploma or GED in Combination With One (1) Year of Life / Work Experience Working With HIV / STI and SUD / COD

Monday - Friday 8am-5pm or 7am-4pm

Position Summary : The mobile HIV / Portable Clinical Care / Peer Outreach Specialist particpates as a member of an interdiciplinary clincial team working with individuals with or at risk of HIV / STIs, SUD, or other mental health and behavioral health conditions, and underserved and / or unsheltered homelessindividuals.

Provides active outreach in the community, including on the streets, in homeless encampments, in parks and other public places.

Develops professional caring relationship to engage individuals into available primary medical care, SUD, BH, harm reduction for HIV / STI / SUD and SDoH services.

Provides education to patients about prevention care, wellness activities and prescribed treatments. Makes connections to specialty serives with partner service agencies.

Assists members in pursuing and achieving their recovery based goals through direct one to one and group psychosocial support, brief clincial interventions, motivational interviewing, active listening, and sharing their personal shared experience to support the client's recovery.

Works under the direct supervision of the team Project Coordinator.

Essential Duties :

Works collaboratively with the clincial team to engage, educate, comminicate, and coordinate care with partcipants, their family, behavioral health, medical and other providers, community resources and others in ensuring that all serices prescribed in the individualized service plan are implemneted.

  • Maintain appropriate knowledge, skill, and certification for this position.
  • Conduct face to face interactions with clients, family or other involved parties in the home, community, and integrated care clincis for the purpose of assessing, maintaining, or enhancing a person's functioning.
  • Establish therapeutic relationships with a variety of patients for the purpose of establishing and reaching appropriate health and treatment goals.
  • Provides direct assistance in finding necessary resources other than covered services to meet basic needs for clients served.

Qualifications

High school diploma or GED in combination with one (1) year of life / work experience working with HIV / STI and SUD / COD.

High level of competency in Microsoft Word, Intermediate Level Excel, Power-Point, Outlook.

  • Strong oral, written and interpersonal communication skills. Ability to assist patients, convey information, resolve issues, cultivate connections, build solid relationships and express empathy throughout the patient experience.
  • Phlebotomy trained and certified preferred, but not required. Willingness to undergo phlebotomy certification is required and needs to be completed within 60 days.

Flexibility and ability to work evenings and weekends if the Director deems it necessary to achieve the objectives of the program.

Willing to work with all cultures and sub-cultures that comprise the population of Maricopa County in a non-judgmental manner.

  • Must have or be able to attain CPR / First Aid certification within 60 days of assuming role.
  • Phlebotomy trained and certified preferred, but not required. Willingness to undergo phlebotomy certification is required.
  • Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policy.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass a TB Test
