IMMEDIATE HIRE
Hours : Monday through Friday 4 pm - 8 pm
Weekends are flexible for community outings.
Assist the individual in his / her as documented in the ISP, Service Plan, etc.
- Teach / assist the client in cooking simple meals.
- Routine hygiene care throughout the day.
- Medication reminder
- Supervise in addressing the ISP / Plan of Care goals and document progress
- Support all opportunities for the growth, learning, and development of the individual
- Complete daily progress notes highlighting person center events
- Complete understanding of home policies and procedures including home rules, disaster procedures, participant's rights, etc.
- Document progress notes before leaving the job site
- Document tracking of ISP goals before leaving the job site
- Ensure the individual is integrated into the community
- Notify the agency of any concerns regarding the individual
- Attend all staff meetings (required), and participate in scheduled training sessions including on-the-job training
- Promote harmonious relationships with other staff members
- Maintain individual's records on-site following policy and procedures
- Other duties as assigned
