Position Summary :
Assure residents balanced nutrition needs due to specific medical conditions with food preferences to achieve agreed upon goals.
Qualifications & Education Required :
- Bachelor’s Degree in Food and Nutrition or Dietetics required
- ADA registered or registration eligible required
- Certified Dietitian / Nutritionist in NYS preferred
- 1 year in long term care preferred
- Ability to interact with elders and care for their needs
- Ability to lead and coordinate team activities

Responsible for nutritional assessments and monitoring of assigned residentsCollaborate with clinical teams to facilitate appropriate nutritional care and care planning on an as needed basisProvide diet counseling to residents and their family members as neededAct as a liaison for policy and procedures pertaining to clinical nutritionServe as a liaison between foodservice and clinical nutrition needs; including menu updates, product updates and changes and coordination of therapeutic and mechanically altered dietary needsHelp with other duties as needed that support the Core Team to fulfill St John’s mission vision and valuesPhysical Requirements :
Walking, standing for lengthy periods of time.Exposure to Conditions :
May provide care to individuals with infectious diseases. May be subject or exposed to experiences that involve physical aggression. Occasional use of complex electrical equipment.
