Job Description : Physician Assistant / Nurse Practitioner

Department : Primary Care | Status : Full-Time Hours may vary including evening and weekends. On-call shared with other providers (1 : 4) | Location : 6374 Lincoln Ave Chicago IL 60659

Reports To : Manager of Clinical Operations / Chief Medical Officer (CMO) | Compensation : 98k-120k

Duties and Responsibilities :

• Understanding and willingness to deliver care in a primary care setting.
• Guide and assist in all aspect of primary care medicine including :

Preventative medical care

Age-appropriate history and physical patient exams
