RN Case Manager - Maternity

A company is looking for an RN Case Manager - Maternity / OB.

Key Responsibilities

Make outbound calls to assess members' current health status

Identify gaps or barriers in treatment plans and coordinate care for members

Provide patient education and support for self-management and healthy lifestyle changes

Required Qualifications

Active, unrestricted RN license in a compact state

3+ years of clinical experience in Maternity / OB

Proficiency in MS Office and ability to navigate a Windows environment

Access to high-speed internet and a dedicated workspace for remote work

Ability to obtain additional licenses if needed
