Job Title : Patient Care Coordinator - Case Manager
Locations : South San Francisco, CA
Duration : 3 + Months (Possible Extension)
Shift : 03.00 AM – 11.00 PM
Rate : $70 per hour (Local) - $80 (Traveller)
Job Responsibilities :
- Responsible for assessing client needs, developing care plans, coordinating services, and advocating for clients to ensure they receive appropriate care and support.
- Strong communication and interpersonal skills.
- Ability to build rapport and trust with clients and their families.
- Excellent organizational and time management skills.
- Problem-solving and critical thinking abilities.
- Familiarity with community resources and support networks.
- Knowledge of case management principles and practices.
- Ability to work independently and as part of a team.
- Proficiency in relevant software and documentation systems.
- Delegates tasks to support staff.
