Case Manager - Full-Time position, (35 Hours Per Week)
Degree : Associates Degree in Human Services, or a related field (Bachelor's degree preferred)
Experience : Relevant experience in crisis intervention and residential settings, reflecting the specific needs of at-risk youth case management at Casey House
POSITION OVERVIEW :
The Youth Services Case Manager is responsible for supporting the Residential Shelter team in Case Management in
addition to demonstrating knowledge of the mission and services of the Agency.

QUALIFICATIONS / EXPERIENCE :
Associates Degree in Human Services (or related field) and one year experience requiredAt least one year experience working with runaway, homeless, and at-risk youth in community-based or residential settingsExperience in crisis intervention, case management, or counseling in a residential setting is a plusValid NYS Driver License and use of personal vehicle during work hoursProficiency in Microsoft applications (i.e. Word, Excel, PowerPoint, Outlook, etc.) and ability to use web-based databasesComfort and professionalism in using email and other digital tools to communicate and document work effectivelyAGENCY EXPECTATIONS :
Reports all suspected child abuse / neglect to NYS Hotline with supervisory approvalAttends and participates in appropriate Agency staff trainings and meetings and program supervisionAdvocates for cultural diversity / sensitivity as appropriateExperience and knowledge on the use of computers, cell phones and a broad understanding of using the Internet and email in the workplaceEffectively communicate with consumers, community partners and Agency staff including accurate documentationParticipate actively in ongoing professional developmentMeets with Program Supervision on a regular, planned basisJOB RESPONSIBILITIES :
Provide ongoing crisis services, positive youth development, trauma informed care, and support for (RHY) runaway, homeless and at-risk youth in an emergency shelterComplete program prescreening interviews and intake paperwork for new residentsInterview, develop, and implement treatment plans in cooperation with individual youthProvide direct case management services to residential and non-residential clientsAssess the needs of youth and make referrals to appropriate community agencies and resources; advocate for youth in community to ensure service needs are being met / maintainedMaintain caseload of youth in residence and complete 30, 60, 90 day follow-up reports for discharged youthEstablish working relationships with referral sources and other related service agenciesMaintain residential and non-residential client files in compliance with regulatory agencies and complete paperwork in a timely mannerKeep records for evaluating and statistical reporting to funding agenciesReview current practices to advocate for change when needed, particularly those needed by RHYParticipates actively in RHY training and educationEducate youth in shelter on various topics including but not limited to HIV, AIDS and other items as determined by program guidelines. Orient youth to group home including resident guidelines, fire safety, etc.Team with program staff in order to provide consistency of servicesTransport clients as appropriateRepresent Agency and Program in the community; conduct school and community presentations for the purpose of educating youth about program servicesProvides outreach for Casey House services to youth, agencies, personnel, families, schools, etc.Attends and participates in appropriate Agency staff trainings and meetingsPHYSICAL / ENVIRONMENTAL DEMANDS :
