Description :

The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs.

The overall goal of the position is to enhance the quality of patient management and satisfaction.

This role promotes continuity of care and cost-effectiveness through the integrating and functions of case management, utilization review, and discharge planning.

The Case Manager must be a highly organized professional with great attention to detail, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies.

Experience in acute hospital settings is required.

EPIC experience is necessary.

Candidates should possess two years of healthcare experience as a Registered Nurse.

This position requires strong skills in care coordination, concurrent review, discharge planning, and navigating admission criteria.

Knowledge of medical necessity and utilization review guidelines is essential.

Weekend shifts are required as part of the role.

Candidates must be able to work collaboratively within an interdisciplinary team.
