Position Overview :

We are seeking an experienced Dialysis Clinic Intake Manager to lead our intake operations and enhance the patient experience at our clinics. The ideal candidate will bring a strong background in case management and social work, along with a passion for serving the community. In this role, you will be responsible for overseeing the intake process, coordinating patient care, and collaborating with our healthcare team to ensure the highest level of service.

Key Responsibilities :

• Manage the daily operations of the intake department, ensuring optimal patient flow and service efficiency.
• Conduct comprehensive assessments of new patients to determine their needs, eligibility, and appropriate care plans.
• Facilitate communication between patients, families, and the clinical team to address concerns and provide support.
• Ensure compliance with all regulatory standards and organizational policies.
• Train, mentor, and supervise intake staff, promoting a collaborative and patient-focused culture.
• Monitor and analyze patient intake data to identify trends and areas for improvement.
