Certified Recovery Support Worker - Harbor Homes, Harbor Care Health & Wellness Center
Summary : The CRSW is responsible for providing a range of case management and recovery support services to adults who are receiving Medication Management for their substance use disorder through Harbor Care's medical clinic.
This position is full time, first shift (8am - 5pm).
Knowledge and Abilities
- Certified Recovery Support Worker (CRSW) certification with the state of New Hampshire
- Ability to work with people experiencing and / or recovering from a Substance Use Disorder
- Convene, facilitate, and manage groups
- Provide recovery support services
- Ability to provide outreach communication (verbal and written) and document contact with clients
- Experience working with culturally and clinically diverse populations.
Responsibilities
- Provide individual case management and recovery support services to participants in the Medication Management program
- Track and document participant engagement in recovery support services
- Facilitate recovery support groups as needed
- To participate in liaison work with other agencies as required
- To participate as a member of an interdisciplinary treatment team
- To participate in regularly scheduled clinical supervision
- To maintain clinical records in accordance with insurance regulations and established clinic policies and procedures
- To participate in staff meetings
- Any and all other duties as assigned

Requirements
Basic Qualifications :
- High School Diploma or GED
- Currently certified in the state of New Hampshire as a CRSW
- Harbor Care is a recovery-friendly workplace. We encourage applicants who have recovery experience to apply.
Preferred Qualifications :
- Bilingual English with Spanish or Portuguese
- Current Driver's License
Knowledge, Skills and Abilities :
- Ability to work with clients of varying backgrounds
- Knowledgeable about different paths to recovery from substance use and able to support clients in their journey
- Organization skills
- Effective time management skills
- Ability to read and write in English
- Self-directed, and ability to take initiative
- Must be able to work in busy settings
Responsibilities
- Meet with clients receiving treatment for substance use disorders either individually or in groups to provide personalized recovery support
- Outreach program participants regularly to provide recovery support and engage participants in their care to prevent them from falling out of treatment
- Participate in weekly integrated care team meetings to review and coordinate client care
- Complete documentation in our Electronic Health Record (EHR) Athena for all services provided
- Collect and enter data on participants into online databases to track program efficacy
- This position is located full time in our medical clinic
