About the Role :
We are seeking a dedicated and compassionate Hospice Registered Nurse to join our team at One Health Home Health and Hospice.
In this role, you will have the opportunity to provide quality end-of-life care to patients in Contra Costa County and Alameda County, making a meaningful difference in their lives during a challenging time.
Responsibilities :
- Provide skilled nursing care to hospice patients in their homes
- Assess patient needs and develop individualized care plans
- Administer medications and treatments as prescribed by the physician
- Provide emotional support and education to patients and their families
- Collaborate with the interdisciplinary team to ensure comprehensive care
Requirements :
- Active Registered Nurse license in the state of California
- Minimum of 1 year of nursing experience, hospice experience preferred
- Strong communication and interpersonal skills
- Compassionate and empathetic demeanor
- Ability to work independently and as part of a team
