Social Worker - Hospice

Description

We are looking for a 32- hour Social Worker to join our Hospice team.

Social Services are provided in accordance with the Hospice Plan of Care. The Social Worker assists the physician and other team members in understanding the significant social emotional factors related to health problems. Participates in the development of the Plan of Care, prepares clinical and progress notes, works with the family, utilizes appropriate community resources, participates in discharge planning and in-service programs, and acts as a consultant to the team and other agency personnel.

Requirements

1. Bachelor's degree in Social Work required.

2. Two (2) years' experience working in a community setting preferred.

3. LCSW or LICSW preferred
