New York Licensed Bilingual Care Manager

A company is looking for a Bilingual Spanish Care Manager - DSNP.

Key Responsibilities

Advocates and educates beneficiaries on services and health benefits

Conducts assessments and develops care plans in alignment with treatment goals

Coordinates care among service providers and documents all encounters in the appropriate systems

Required Qualifications

NYS RN or LCSW / LMSW (any state) for Medical Care Management

3 years of experience in a mental / behavioral health or addictions setting for Behavioral Health Care Management

1 year of pediatric clinical field experience for PEDS positions

Fluency in Spanish, Korean, Mandarin, or Cantonese preferred

Experience managing member information in a shared network environment
