Job Description :
We're seeking an Experienced Home Health Nurse with a minimum of 1 year of home health nursing experience. Proficiency in Axxess software and top-notch charting skills are a must.
You'll play a crucial role in delivering high-quality care and maintaining accurate documentation.
Key Responsibilities :
- Conduct thorough health assessments in clients' homes.
- Develop personalized care plans for optimal outcomes.
- Administer medications and skilled nursing care.
- Monitor patient progress and adjust care plans as needed.
- Educate patients and caregivers.
- Maintain precise electronic records using Axxess software.
- Communicate effectively with patients, families, and healthcare professionals.
- Ensure compliance with home health regulations.
Qualifications :
- Minimum 1 year of home health nursing experience.
- Proficiency in Axxess software with at least one year of hands-on experience.
- Strong charting and documentation skills.
- Current nursing license in Illinois.
- Excellent communication and interpersonal skills.
- Compassionate and dedicated to high-quality care.
- Ability to work independently and collaboratively.
- Reliable transportation and valid driver's license.
