Texas Licensed Case Manager Nurse

A company is looking for a Registered Nurse II - Case Manager.

Key Responsibilities

Manage continuity of care for beneficiary populations through comprehensive assessments and care planning

Coordinate follow-up care services and facilitate communication among healthcare team members

Develop and implement tools and strategies to support case management and improve patient outcomes

Required Qualifications

Baccalaureate Degree in Nursing from an accredited institution

Certification in case management or related fields (e.g., CCM, CDMS, CRRN)

One year of nursing experience post-graduation

Current, unrestricted Registered Nurse license
