This role is ideal for a physician passionate about delivering value-based care and improving patient outcomes for individuals with kidney disease in their homes and at dialysis facilities.

The successful candidate will provide comprehensive nephrology care and collaborate with interdisciplinary teams to develop and implement value-based care initiatives. This is a pivotal role that requires a commitment to patient-centered care and the ability to drive key clinical metrics that improve the quality of care and reduce costs.

Qualifications

Required :

• Medical degree from an accredited medical school.
• Board certification in Internal Medicine and Nephrology.
• Current state medical license and DEA registration without restrictions.
• Strong clinical skills and experience in managing patients with kidney diseases across various settings, including inpatient, outpatient, and dialysis.
• Commitment to delivering patient-centered care and promoting shared decision-making.
• Excellent communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and healthcare teams.
• Must be willing and able to obtain hospital privileges at required facilities.
