Virtus Healthcare Staffing is seeking a skilled and compassionate Lead Nephrology Specialist to join a partner's team in Houston, TX. This role is ideal for a physician passionate about delivering value-based care and improving patient outcomes for individuals with kidney disease in their homes and at dialysis facilities.
The successful candidate will provide comprehensive nephrology care and collaborate with interdisciplinary teams to develop and implement value-based care initiatives. This is a pivotal role that requires a commitment to patient-centered care and the ability to drive key clinical metrics that improve the quality of care and reduce costs.
Qualifications
Required :
- Medical degree from an accredited medical school.
- Board certification in Internal Medicine and Nephrology.
- Current state medical license and DEA registration without restrictions.
- Strong clinical skills and experience in managing patients with kidney diseases across various settings, including inpatient, outpatient, and dialysis.
- Commitment to delivering patient-centered care and promoting shared decision-making.
- Excellent communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and healthcare teams.
- Must be willing and able to obtain hospital privileges at required facilities.
Preferred :
Experience with value-based care models, quality improvement methodologies, and population health management initiatives.Key Responsibilities
Patient Care & Clinical Excellence
Provide comprehensive nephrology care to patients with acute and chronic kidney diseases at home and at dialysis facilities.Utilize evidence-based practices and clinical guidelines to optimize the management of kidney disease and associated comorbidities.Engage in patient education and shared decision-making to empower patients in managing their kidney health.Actively participate in multidisciplinary care conferences and meetings to review complex cases and collaborate on treatment plans.Operational Oversight
Collaborate with interdisciplinary teams to develop and implement value-based care initiatives aimed at improving patient outcomes and reducing healthcare costs.Utilize technology and data analytics to track and monitor patient outcomes, identify areas for improvement, and drive continuous quality improvement.Drive key clinical metrics that impact kidney care, such as home dialysis rate, permanent access, planned dialysis starts in the outpatient setting, and conservative kidney management.Why Join Us?
