Catholic Charities of Orange, Sullivan & Ulster, is looking for a full-time Case Manager Outreach Worker for their Orange and Sullivan program!

The hourly rate for the position is $23.00. / hour.  This position comes with benefits.

Summary :

This position has multiple responsibilities with both the Social Safety Net and Prevention

Services departments. In this role, the worker will provide direct case management services to the

needy and disenfranchised in Orange and Sullivan County. It is expected that this will be

accomplished through advocacy, providing information, referral, and short-term case management. The position will work collaboratively, connecting Social Safety Net Services with our  Substance  Use  Disorder  Departments  and  Prevention  Department.   The  Case Manager / Community Outreach Worker is expected to be organized, paying close attention to detail, and working steadily and productively. Be able to work independently and with local community-based agencies and

school-based personnel. The Case Manager / Community Outreach Worker will assist the Director of Prevention and the Director of Human and Social Services with any reporting requirements monthly and yearly to ensure all goals / objectives are met. In addition, the candidate will assist the Case Management Department with outreach events to connect families to the services they need in the community of Orange and Sullivan.

Essential Duties and Responsibilities include the following : other duties may be assigned.

1. Conduct initial intake utilizing the Apricot platform database system.

2. Actively participate in system-wide data sharing through integrated digital referral systems

(Unite Us) or other means.

3. Utilize basic Substance Use Disorder Assessments and a brief bio-psychosocial intake to ensure

all needs are identified comprehensively. Provide education to the client and family system

regarding adverse childhood effects, effects on the fetus when drug use or alcohol use is used by

the mother and present in the household, and collaborate with the SUD clinician to provide

comprehensive wrap-around services.

4. Prioritize intakes and ensure warm handoffs throughout the CCOSU system and community partners. Ensuring all intake, screen tools, and notes are completed thoroughly and on time.

5. Attend the Parent-Child Program training and develop service delivery with efficacy.

6. As part of the Interdisciplinary team, ensure that all program participants receive the full

benefit of CCOSU resources.

7. Maintain current and accurate documentation of services provided to clients.

8. Answer telephones and provide information to callers.

9. Develop and maintain working relationships with Orange and Sullivan Counties providers.

10. Attend community and service provider meetings as needed.

11. Maintain an updated Community Resource list / guide to use as a referral source for clients

needing it.

12. Participate in outreach activities on the weekend and after hours, such as community tabling

events, community outreach, pop-up pantry outreach, food pantry outreach, and other case management and prevention department activities.

13. Provide community outreach to ensure access to the program and community education about

services related to the determinants of health.

14. Participate in regular supervisory sessions with immediate supervisors.

15. Attend agency’s in-service training, case conferences and formalized coaching from the

Parent-Child Program.

16. Travel to meet clients at other CCOSU offices.

17. Attend monthly SALT (Sullivan Allies Leading Together) coalition meetings and sit on

appropriate prevention-related committees and Case Management.

18. Attend various community events to grow the prevention department and provide safe drug

disposal information to community members.

19. Work collaboratively with local community-based agencies / organizations.

20. Assist with school-based prevention programming as needed.

21. Assist the Prevention and Case Management department with planning and implementing community events and trainings.

22. Create / maintain records of contacts in the community with regular email communication.

23. Assist in the development of youth / community outreach projects, including creating and

preparing outreach materials such as flyers / forms, building social media sites, creating content,

building social media engagement, and monitoring / recording engagement.

24. Assist with monthly reporting requirements.

25. Ability to be flexible within two departments (Case Management and Prevention)

26. Other duties as assigned by the supervisor or demands of the program and grant funders.

Position Type and Expected Hours of Work :

This is a full-time position. Days and hours of work are generally 9AM to 5PM Monday -Friday.

Additional hours are required on a regular basic and candidate will need to be flexible and able to

work evening and / or weekends to meet program deadlines, attend outreach events or client needs.

This position requires the candidate to travel to multiple sites as needed between Orange and

Sullivan County to assist both department case management and prevention.

  • Comfortable with public speaking.
  • Comfortable in group settings of all ages.
  • Basic understanding of client engagement and de-escalation techniques
  • Prepare comprehensive progress notes and service plans
  • Traveling to Multiple sites 40% of the time.
  • Remain stationary and use a computer at least 60% of the time.
  • Evening and weekend flexibility are required.
  • Operate computer, telephone and other office productivity equipment, such as photocopier,

scanner, laminator, shredder, and printer.

  • Ability to lift at least 50 pounds.
  • Sitting, standing, walking, talking, bending, lifting, and stooping are required.
  • Climbing stairs.
  • Qualifications

    Education and / or experience required :

  • Associate’s degree in related field of social services preferred.
  • High School Diploma or Equivalency required with at least two years’ experience working in case
  • management with families.

  • Two years of office administration experience with MS office expertise.
  • Knowledge of various social media platforms.
  • Must possess a clean NYS Driver’s License and maintain it clean throughout employment.
  • Skills, Licenses, and / or competencies required :

  • Bilingual (English / Spanish) preferred.
  • Knowledge of Social Services available in Orange and Sullivan counties preferred.
  • Computer literate with knowledge of all aspects of Microsoft Office.
  • Excellent interpersonal and communication skills, work as part of a team and ability to work
  • independently and comfortable with public speaking.

  • Excellent oral and written skills to effectively communicate with all levels of staff, outside
  • agencies and the community.

  • Crisis intervention skills and empathy in facilitating client service delivery.
  • Good organizational and time-management skills.
  • Basic understanding of client engagement and de-escalation techniques.
  • Operate computer, telephone and other office productivity equipment, such as photocopier,
  • scanner, laminator, shredder, and printer.

  • Ability to work with a wide range of people, including youth, parents, community leaders, and
