Facility : Home Health & Hospice

Schedule : 5x8 Days (8 : 00 AM – 4 : 30 PM), 40 hours / week

Assignment Details

Start Date : ASAP (Urgent Need)

Assignment Length : 16–32 weeks

Hours : 40 hours per week, 8-hour day shifts

Weekends : Every weekend required

Holidays : Holiday RTO cannot be guaranteed

Requirements

Experience : Minimum 2 years (Hospice or End-of-Life Care preferred)

License : Active Vermont LNA license (required for offer)

Certifications : Current BLS certification

EMR System : Interactive Scheduler / Netsmart

Transportation :

Public transportation not available — reliable personal or rental vehicle required

Vehicles should have all-season or snow tires (November–April)

Additional Information

No unexplained employment gaps over 90 days (must provide explanation if applicable)
