Core Health Provider Services, Inc has an employment opportunity for a LPN in Marietta, GA 30067. The LPN provides direct nursing care to individuals with intellectual and developmental disabilities (IDD) in accordance with DBHDD standards. This includes administering medications, monitoring health conditions, supporting behavioral health needs, and collaborating with interdisciplinary teams. We are looking for individuals who have a sincere desire to help others. Our mission is to serve adults with intellectual / developmental disabilities, by providing specialized services. Supports are individually planned and tailored to meet the specific needs of the individuals we serve.
Position Details :
Days : Monday, Tuesday, Wednesday, Thursday, Friday
Time : 8 hour shifts
ESSENTIAL DUTIES AND RESPONSIBILITIES
Administer prescribed medications and treatmentsCommunicates with RN, DDP, and interdisciplinary team as neededParticipates in team meetings and health reviewsDocuments all communication with physicians and familyMonitor and document changes in individuals physical and behavioral healthMeasure and record vital signs (temperature, blood pressure, pulse, respiration)Assist with personal hygiene, mobility, and daily living activitiesCollaborate with case managers, physicians, and behavioral health professionalsParticipate in Individual Support Plan (ISP) meetings and implement care plansMaintain accurate and timely documentation in compliance with DBHDD protocolsRespond to emergencies and adverse reactions appropriatelyEducate individuals and caregivers on health maintenance and medication adherenceAdministers medications per physician orders and documents in MARMonitors and documents vital signs, health status, and behavioral changesProvides wound care, catheter care, and other delegated nursing tasksReports significant health changes to RN or physician promptlyParticipates in ISP development and implementationDocumentationRequired Qualifications :
The successful candidate must :
Be at least 21 years of ageAt least one year of nursing experienceGraduate of an approved professional nursing programHave dependable transportation, valid drivers license, and valid car insuranceBe able to satisfactorily pass a criminal background checkProvide a clean 7 year driving record, with no suspensions due to moving violations in past 3 yearsMust obtain a TB testCurrent CPR / First Aid certificationCurrent physicalCurrent Georgia license to practice as a Licensed Practical Nurse (LPN)Salary : $32.00 - $35.00 / hour
Education :
Current LPN licensure without restrictions or provisions in the state of GA (Required)Completion of DBHDD-required orientation and annual trainingCPR / First Aid certification (current)HIPAA and infection control training completed
