We're looking for a Care Manager to join our behavioral health team. This role focuses on providing case management and support services to individuals with moderate to severe mental health needs. You'll work directly with clients, families, and providers to ensure care is coordinated and that each person has access to the resources and support they need.

Responsibilities / Job Duties :

• Manage a caseload of 20-25 clients, including both youth and adults
• Conduct assessments and develop individualized care plans
• Coordinate with mental health, medical, and social service providers
• Support clients in following treatment plans, attending appointments, and accessing resource
• Provide education around self-management of health conditions and lifestyle choices
• Link clients to community services, such as housing, healthcare, and social supports
• Document all services accurately and in a timely manner using an electronic health record system
• Participate in team meetings and consultations
• Maintain billable service expectations (35 hours per week)

Minimum Education and Experience Qualification Requirements :

Schedule / Shift : Monday - Friday 830am - 5pm (30 min lunch)
