Jamaica Hospital Medical Center , a leading healthcare facility in Queens and member of the MediSys Health Network, is seeking a dedicated Case Manager (RN) to join our Case Management Department.

Key Responsibilities
- Conduct comprehensive assessments of medical, psychological, and social needs.
- Develop and implement individualized care plans with patients, families, and interdisciplinary teams.
- Coordinate patient discharges, ensuring safe transitions to home or other care settings.
- Document care in the EMR accurately for compliance and quality reporting.
- Collaborate with physicians, nurses, social workers, rehab, pharmacy, and other professionals.
- Educate patients and families on medications, follow-ups, and community resources.
- Manage high-risk and complex patients, proactively removing barriers and preventing readmissions.
- Advocate for patients and coordinate access to financial, social, and community resources.
- Ensure compliance with Joint Commission standards, CMS regulations, and hospital policies.
- Address unexpected challenges using problem-solving and crisis intervention skills.

Requirements
