Job Description

POSITION SUMMARY : The Program Specialist implements, oversees, and ensures that the services provided in the Community Living Program (CLP) program adhere to applicable regulations and that the services which are provided meet the needs of the individuals receiving services, according to the Individual Support Plan (ISP). The Program Specialist develops, completes, and oversees the completion of required documentation; assesses individual needs; coordinates and secures services; trains staff; attends and leads meetings; communicates with all team members, including family members; and ensures the continuous development and monitoring of programming and services to people for whom they are responsible. The Program Specialist ensures that services provided are fully compliant, meeting the needs of individuals receiving services and are consistent with high quality care standards and best practices in service delivery.

Duties and Responsibilities

  • Ensure that Program Coordinators (PCs) and Direct Support Professionals (DSPs) comply with implementing and / or overseeing processes and activities, including a system of safe practices, inspection readiness always, and HIPAA compliance by conducting ongoing training and audits to support these compliance measures
  • Ensure sites operate in accordance with the organization's quality standards for safety, cleanliness, comfort, and state licensing and accreditation standards by, for example, leading staff meetings to train staff with the Program Coordinator and report any deficiencies back to the appropriate director of operations
  • Develop and maintain individual electronic and paper records to ensure accurate documentation
  • Conduct all admissions, transfers, and discharge activities including but not limited to completion of admission packet, coordination of initial ISP meeting, completion of transfer documentation and records, preparation of discharge letter, etc. to assist admissions / transfers, etc.
  • The Program Specialist provide details regarding the services available within the agency to the team and the Service Coordinator (SC) to ensure accurate documentation
  • Contribute to the development of outcomes specific to the services provided to ensure the outcomes are appropriate for each individual's needs
  • Assist with the establishment of units, start / end dates, etc., for identified services included in the ISP and confirming their accuracy within the ISP to ensure accurate billing
  • Report changes in individual service needs / preferences which may cause the need for changes to the ISP or effect the provision of services to ensure the individual is receiving all appropriate and accurate services
  • Review provided information related to the individual's receiving services including the Person-Centered Planning Tool (PCPT), previous ISP(s), available assessments, and evaluation data, as appropriate / relevant the individuals needs
  • Notify the support coordinator and requesting an Interdisciplinary Team (IDT) meeting be scheduled whenever there is a notable change in the individual's status regarding any medical, clinical, or operational needs in the residential and Day Habilitation Programs
  • Participate in the development and reviewing of the ISP, ISP annual update, and ISP revision and ensuring completeness, content accuracy, and that the ISP reflects the services and supports needed are included and have been provided
  • Attend all ISP meetings to ensure all pertinent information is conveyed and documented by support coordination
  • Report content discrepancy and changes related to the individual's needs to the Service Coordinator (SC), as applicable, and plan team members, by using the Department of Developmental Disabilities (DDD) issued worksheets to ensure the content is documented
  • Keep documentation of all communication with the SC of changes that must be made to the ISP, including changes to outcomes based on a need or revision to the assessment to ensure accurate information is documented
  • Assist in developing outcomes for individuals based on appropriate assessments provided by the clinical team and training all staff in the implementation of outcomes and strategies to ensure training is accurate
  • Ensure that all services / supports provided relative to ISP outcomes are reviewed with the individuals and that the ISP includes community involvements such as residential and day program outings, civic-minded opportunities, and membership in national or local organizations etc.
  • Coordinate and complete the training of DSP in the content of ISP needs relevant to everyone receiving services to ensure that accurate content is trained correctly.
  • Liaison with all team members, including individual's family, SCs, medical personnel, day program personnel, residential, by all means of communication to ensure all team members are updated with the most recent and accurate information
  • Aid in the resolution of problematic situations that may occur for all Elwyn New Jersey residential and day habilitation individuals
  • Promptly report any infractions of failure to follow established Elwyn NJ policies and procedures with DDD, HIPAA, and applicable state / local / federal regulations at all times to designated and authorized person outlined in each applicable policy or regulation
  • Maintain current state-mandated training and certifications
  • Follow all current policies and procedures of Elwyn New Jersey
  • Cooperate with Elwyn New Jersey and the State of New Jersey Department of Human Services, or any other authorized investigating body during an inspection or investigation
  • Perform cardiopulmonary resuscitation (CPR), safe crisis management, and medication administration using agency-trained protocols
  • Provide on-call coverage
  • Perform other related duties as assigned
  • Education / Experience / Skills Requirements

  • Possess at least a bachelor's degree in a human services field and at least two (2) years experience working directly with persons with intellectual and development disabilities (IDD), OR sixty (60) college credits in human services field with two (2) years case management experience and additional two (2) years of direct support with an IDD population, OR one (1) year IDD case management experience with five (5) years of direct support with an IDD population
  • Non-profit human service experience highly preferred
  • Demonstrated ability to coordinate teams and individual staff
  • Demonstrated ability to work effectively as part of a team
  • Knowledge of applicable regulations including but not limited to Community Care Program and Supports Program Manual, and applicable New Jersey State regulations
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
  • Demonstrated strong attention to detail
  • Demonstrated strong time management and organizational skills
  • Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
  • Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
  • Ability to perform CPR and safety care techniques in accordance with agency trained protocols
  • Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint; report-writer experience preferred
  • Experience using / knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred
  • Must have current, valid driver's license in state of residence, 3 years driving experience in the United States, and acceptable driving record
