Position Summary

We are seeking passionate and dedicated Registered Nurses to provide skilled home health services to adult patients. You will have the flexibility to customize your patient load based on your availability and work-life balance. Whether you are an experienced home health RN or a newly licensed nurse eager to learn , we welcome you!

Languages in demand : Ukrainian, Russian, Polish, Armenian, and / or Spanish — a huge plus!

What You’ll Do

Perform skilled nursing visits, including assessment, diagnosis, and treatment according to the plan of care

Complete OASIS assessments and documentation in compliance with Medicare and agency standards

Educate patients and families on disease management, medication administration, and self-care techniques

Collaborate closely with physicians, therapists, and other healthcare professionals to ensure continuity of care

Maintain accurate and timely clinical documentation

Qualifications

Current Illinois RN license (required)

Valid driver’s license, auto insurance, and reliable transportation

Home health experience preferred, but new graduates are encouraged to apply

Strong communication and interpersonal skills

Ability to work independently and manage time effectively
