LVN Clinical Intake
Job Type : Full-time
Salary : $50,000
Schedule : Monday – Friday, 8 : 00 AM – 5 : 00 PM (in person)
On-Call Rotation : Every 3 weeks, 9 : 00 AM – 6 : 00 PM (in-office during rotation)
Primary Responsibilities :
- Review and process new referrals, including orders and clinical documentation.
- Build and maintain strong relationships with referral sources.
- Manage inbound inquiries from clients, family members, and field team members.
- Coordinate patient care with field teams to ensure smooth delivery of services.
- Oversee resource coordination, including specialty pharmacy, DME, supplies, and lab arrangements.
- Provide hands-on assistance and troubleshooting as needed.
- Partner with sales to coordinate acceptance of new patients.
- Maintain accurate documentation and participate in the clinical on-call rotation.
- Gather and review client needs to support scheduling.
- Deliver exceptional service to both internal and external stakeholders.
- Promote continuity, safety, and quality in all aspects of client care.
Experience and Education Requirements :
High school diploma or equivalent required.LVN license (Must have for a least 1 year)Proven ability to communicate effectively with referral sources and clients.Demonstrated experience in de-escalation, customer service, and high-volume call management.Proficient with web-based systems, Microsoft Outlook, Word, and Excel.Minimum Skills and Abilities :
Excellent communication and interpersonal skills.Strong organizational skills and attention to detail.Ability to perform well under pressure and manage multiple priorities.Commitment to delivering high-quality care and client support.Work Environment : This position is based in a fast-paced office environment with frequent phone interaction. Physical requirements include sitting or standing for extended periods, occasional bending and stooping, and lifting up to 10 lbs.
