Job description11 / 4 (7a) - 11 / 10 (7a) 11 / 27 (7a) - 12 / 2 (7a) 12 / 2 (7a) - 12 / 8 (7a) 12 / 24 (7a) - 1 / 5 (7a) 1 / 22 (7a) - 1 / 27 (7a) 2 / 18 (7a) - 2 / 24 (7a) 3 / 19 (7a) - 3 / 24 (7a) 4 / 16 (7a) - 4 / 21 (7a) Inpatient only Patient volume varies - 3 consults per day, 2 procedures per day Trauma level II 18 ICU beds, 18 tele beds, and 10 CMU beds APP support - APP's- M-F 1-2 APP's are staffed during the day, weekends- 1 APP for 5 hours / day Interventional cardiology procedures required TEE's would be highly preferred. Balloon pumps, coronary stents, IVUs and IFR are important. STEMI volumes vary greatly. In 2020 they averaged 3 STEMI activations per week Flights - $750 / round trip cap (to include all taxes and fees) Lodging - $179 / night cap for hotels (to include all taxes and fees). Preferred Lodging : Candlewood Suites Boise Towne Square, an IHG Hotel. Rental Car - To be reimbursed by facility (not to be used with personal car / mileage reimbursement). $50 / day cap (including all taxes and fees) regardless of car class. SUV / upgrades over cap will need to be preapproved (will only be approved if needed for inclement weather). Mileage - IRS Standard Rate to be reimbursed by facility (only to be used with personal vehicle, not to be used with rental car). Malpractice (must be included in the rates) Orientation 24 Hour On Call ($4800 rate cap with 10 hours gratis) Holiday 24 Hour On Call Weekend Call Holiday Weekend Call Call Back Holiday Call Back Conversion fee ($25k rate cap) MD diploma Internship / residency / fellowship certs All medical licenses DEA Board Certification Life support cards- BLS Current and previous certificates of insurance Driver s license Professional color photo / headshot CMEs for the past 2 years Immunization records including TB, Flu, HepB, Tdap, Varicella, and COVID References Case logs Please Note : Trinity Health does not reimburse for any application or credentialing fees. Board Certified Interventional Cardiology - [REQUIRED] Active ID license or active IMLC (LOQ at time of name clear) - [REQUIRED] Clean malpractice and no background - [REQUIRED] Local candidates, no flights - [HIGHLY PREFERRED] Must meet perm rate cap of $25k - [REQUIRED] Must have COVID vaccine in order to work at this facility - [REQUIRED] - Verbal confirmation required at time of name clear Must be able to cover most / all of these open dates - [REQUIRED]
