Pay range Salary : $29.76-$38.39
Essential Functions :
- Determines eligibility, acceptance, and placement for individuals with Substance Use Disorder issues.
- Serves as an active, supportive team member and adheres to the team approach philosophy in the workplace.
- Works closely with other staff in the implementation and adherence of standards set forth in both quality improvement and utilization management systems.
- Completes and maintains appropriate records which meet the IHS / Tribal mental health and alcohol and drug standards, State of Oregon standards for mental health and alcohol and drug programs, AAAHC accreditation standards, and Program policy.
- Maintains records to meet program documentation, quantitative and peer review standards, clinical and administrative supervisor expectations.
- Participates in outreach, determines eligibility, and makes referrals for individuals who may be interested in residing in the Women's Transitional Living Center.
- Utilizes crisis intervention techniques to maintain individual and community stability.
- Monitors each individual's risk of harm to self, others, and property, and refers individuals to appropriate services, department(s) or agencies.
- Provides treatment as reflected in Individualized Treatment Plans that is commensurate with clinician education and licensure as well as individualized resident transition plans.
- Reports and documents possible side effects, medication effectiveness and compliance, and consults appropriately.
- Teaches skills in a group setting in an interactive manner that engages individual participation.
- Works within the Women's Transitional Living Center. May do other home visits as necessary. Authorized to serve both eligible and non-eligible beneficiaries in an outreach capacity both on and off site.
- Makes court appearances and provides testimony consistent with confidentiality obligations when requested by the Tribe.
- Summarizes individual's progress toward goals and objectives.
- Re-assesses effectiveness of and works with team members to adapt treatment plans in a timely and appropriate manner.
- Assesses the need for involvement of family or significant others in the development of the plan of care and communicates recommendations to the treatment team. Assists with implementation as appropriate.
- Provides knowledge of community resources to individuals.
- Coordinates care internally and externally and acts as a liaison with community providers and systems.
- Advocates on behalf of individuals to coordinate appropriate services related to treatment goals and outcomes.
- Facilitates stability and independence, while empowering individuals.
- Participates or facilitates effective discharge planning and treatment closure.
- Actively participates in clinical case staffing, in-service training, and other meetings.
- Operates as lead in maintaining and improving operational workflows of the Women's Transitional Living Center as well as provides a high level of organization and oversight.
- Performs work in a fast-paced and sometimes stressful environment and must multi-task to complete work assignments.
- Coordinates services such as medical, dental, employment, housing, etc., for individuals.
- Transports individuals in a company vehicle for the purpose of accessing resources in support of transition back into the community, i.e. resources to assist with housing, employment, education, medical / dental care, recovery, and cultural activities.
- Performs case management, assessment of individual needs and provides referrals for services, when appropriate.
- Monitors clinical progress and completes chart forms and progress notes for case file.
- Prepares written status reports to probation officer, courts, and referents and writes and completes weekly reports.
- Participates in weekly clinical staff meetings and weekly individual supervision. Attends scheduled grant team project meetings and assists with project reporting and project evaluation.
- Performs a variety of other duties as assigned by the supervisor.
Education, Experience, and Licenses :
High School Diploma or GED is required. Associate's Degree is Human Services or a closely related field of study is preferred.Minimum of three (3) years of work experience in alcohol / drug or social services.OR equivalent combination of education, training and experience.Driving Requirements :
Must maintain a valid Oregon driver's license or obtain one upon hire, qualify for the Tribe's vehicle insurance, and provide proof of personal vehicle insurance.Qualifications, Registrations, and Certifications :
Certification as Certified Alcohol and Drug Counselor required (CADC - I, II, or III) or able to obtain within six (6) months of hire.Requires First Aid and CPR certification or ability to obtain certification within 60 days of hire.Maintains continuing education requirements for certification, completes need skills training, ensures CPR / First aid card is up to date and maintains certification / licensure and clinical skills.Requires two (2) year's abstinence from alcohol and chemicals if in recovery.Previous experience in a transitional living environment preferred.Must be familiar with the range of community-based self-help support services.Must have one (1) year of experience in domestic violence services.Must be able to utilize strong methods of self-care and stress management.Must be able to demonstrate a basic understanding of counseling techniques and to build appropriate relations with individuals.Requires working knowledge and proficiency in use of office equipment and computer software such as Microsoft Products.Requires excellent verbal and written communication skills, and the ability to write daily reports and prepare correspondence as required.Ability to speak effectively before groups of clients, families and employees of the organization required.Knowledge of Native American culture required; Grand Ronde Tribal culture knowledge preferred.Environmental Factors and Conditions / Physical Requirements :
Continually requires the ability to express or receive detailed information or important instructions through oral communication.Continually requires working with fingers rather than the whole hand or arm. Requires repetitive movement of wrists, hands and / or fingers. Often requires walking or moving about to accomplish tasks.Requires standing and / or sitting for sustained periods of time, raising objects from a lower to a higher position and / or moving objects horizontally.Occasionally requires lifting of up to 30 pounds.Due to the nature of the interactions with other employees and the public we serve, employees may be required to provide documentation of, or receive, certain health vaccinations.This position is frequently subject to inside environmental conditions which provide protection from weather conditions, but not necessarily from temperature changes, and is occasionally subject to outside environmental conditions.May be called upon to serve as a Disaster Reservist during emergencies.Local travel is occasionally required. Travel outside of the state is infrequently required.Background Check :
