Position Overview
Legacy Staffing is seeking a compassionate and skilled Occupational Therapist to join our client’s home health team. This role is responsible for delivering high-quality occupational therapy services in alignment with each patient’s individualized plan of care, as directed by their primary care provider. The Occupational Therapist will also supervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides in accordance with Medicare, Medicaid, and applicable state regulations.
This is a field-based position requiring travel throughout Kern County. A company vehicle is provided.
Key Responsibilities
- Conduct comprehensive patient assessments, including OASIS documentation for Medicare / Medicaid patients.
- Perform evaluations (initial, re-evaluations, discharge) and develop personalized care plans.
- Deliver occupational therapy services focused on improving ADLs and IADLs, physical and cognitive function, and patient independence.
- Recommend and fit orthotic / self-help devices as needed.
- Evaluate home environments for safety and recommend equipment.
- Supervise COTAs and Home Health Aides per regulatory guidelines.
- Maintain accurate, timely documentation and communicate changes in care plans to providers and team members.
- Participate in staff meetings, case conferences, and training sessions.
- Support discharge planning and contribute to the agency’s QAPI program.
- Adhere to infection control protocols and agency policies.
- Ensure compliance with all federal, state, and local regulations.
Qualifications
Licensed Occupational Therapist in the state of practice (or meets educational and certification requirements if licensure is not applicable).Graduate of an accredited Occupational Therapy program (ACOTE / AOTA).Eligible for or has passed the NBCOT certification exam.Minimum 1 year of acute healthcare experience preferred.Home health experience preferred.Strong communication, documentation, and organizational skills.Ability to lift / move up to 30 lbs and perform physical tasks required for patient care.Compensation (Occupational Therapist – Home Health, PPV) :
Per-visit : Eval $100; DC / Re-eval $90; Standard Visit $65; Conference $65 (paid at these rates when documentation is submitted by end of next day).Late documentation : Eval $65; DC / Re-eval $55; Standard Visit $42.50.Drive & admin time : $21.00 / hr (e.g., driving, scheduling, calls).Paid rest breaks : Paid at the Average Hourly Rate earned that week.Overtime : 1.5× the Regular Rate for the week.Out-of-area premium : +15% per 30 miles from the starting point (e.g., 60 miles = +30%).Benefits Upon Permanent Hire
