In Holyoke, MA as part of the Children’s Behavioral Health Initiative (CBHI) facilitate the process of (ICC) services.  Complete youth assessments that determine strengths, needs, and eligibility for the service. Further develops and facilitates Care Planning Team meetings as well as risk management / safety plans when appropriate.  Responsible for coordinating all team member information and meetings, while providing the team with necessary updates that ensure a unified approach in an effort to achieve the family prioritized vision and goals.
