As a Physical Therapist in Home Health, your role will involve conducting comprehensive assessments and evaluations of patients' care needs, focusing on functional status, mobility, endurance, pain management, and other medical conditions. Your expertise will guide the creation of effective treatment plans and interventions aimed at improving patient well-being while monitoring their progress.

Key Responsibilities :

• Assess and evaluate each patient's functional status and physical therapy needs, collaborating with physicians to develop tailored therapy plans.
• Deliver services in accordance with state laws and the established plan of care, coordinating closely with fellow healthcare team members.
• Monitor and report on the patient's response to treatment and any changes in their condition.
• Implement treatments to alleviate pain, restore function, and maintain maximum physical capability following accepted practice standards.
• Educate the healthcare team, patients, and their families about the treatment plan, functional limitations, and progress towards goals.
• Coordinate patient visits as per the Plan of Treatment, completing detailed progress notes after each visit in line with LHC policy.
• Conduct thorough patient evaluations and develop the Physical Therapy plan of care according to state-specific guidelines.
• Communicate evaluation outcomes, set goals, and outline anticipated treatment frequency.

Required Qualifications :

The compensation for this position ranges from $34.23 to $61.15 per hour for full-time employees, based on various factors including local labor markets and experience.
