Description :

The nurse will primarily provide care for a geriatric population.

Common diagnoses include heart disease, chronic respiratory diseases, CHF, COPD, pneumonia, dementia, Alzheimer’s, depression, stroke, cancer, kidney disease, diabetes, liver disease, wounds, joint replacements, and surgery.

Daily responsibilities will include participation in a 15-minute huddle call.

Weekly IDT meetings will be held at the office.

Monthly all-staff meetings are scheduled.

The nurse will not manage vents but may occasionally use CPAP and BiPAP as patients self-manage.

The visit schedule will include seeing 5-6 patients per day, totaling 25-30 points per week, including routine and OASIS visits.

An administrator will always be available after hours, during weekends, and holidays.

Three certified nursing assistants will be available for support.

The work schedule is Monday through Friday from 8 : 00 AM to 4 : 30 PM, with some flexibility in end time.

Weekend work will only be required during on-call shifts, which occur once every six weeks.

Holidays require availability for at least one major and one minor holiday.

Floating between home health and hospice visits will be necessary.

Solid color scrubs without patterns or logos are required, along with closed-toe, non-slip shoes.

All clinicians must clock in and out while providing care.
