We’re seeking a dedicated and compassionate Doctor of Physical Therapy (DPT) to join our Home Health team full-time. Key Responsibilities

• Provide skilled home health physical therapy services, including Start of Care (SOC) assessments, evaluations, re-assessments, follow ups, and discharges.
• Develop and implement individualized treatment plans based on physician orders and patient goals.
• Deliver evidence-based interventions to help patients improve mobility, function, and independence.
• Monitor and document patient progress, adjusting plans as necessary to achieve optimal outcomes.
• Educate patients and caregivers on exercises, mobility, safety, and discharge preparation.
• Collaborate with physicians, nurses, and the interdisciplinary team to ensure coordinated and high-quality care.
