PATH is seeking master's level social services professionals to join our Orange County Interim Housing team as a Clinical Case Manager.

This position will provide therapeutic and case management services to support the guests residing at PATH's Yale Navigation Center in Santa Ana improve their wellness and obtain housing stability.

ABOUT THE YALE NAVIGATION CENTER

The Yale Navigation Center is a bed interim housing site that provides shelter and supportive services for individuals experiencing homelessness in Orange County.

PATH's Interim Housing programs provide low-barrier, high-tolerance settings for program participants, referred to as guests, who are experiencing homelessness and have a variety of complex health, functional and social needs.

These home-like environments are coupled with support and case management services to assist guests with developing a plan to break the cycle of homelessness, stabilize in housing and reintegrate into the community.

WHAT IS A CASE MANAGER?

Case Managers help Interim Housing guests break the cycle of homelessness by assisting them with the establishment and pursuit of a permanent housing plan through accessing and maintaining necessary support services in the community.

Responsibilities of the Case Manager include :

  • Perform initial comprehensive biopsychosocial assessment; develop a plan of intervention that addresses identified needs for housing, ongoing case management services, substance abuse treatment, mental health services, and coordination of needed community services.
  • Collaborate with the client to develop an individualized treatment plan that addresses short- and long-term goals.
  • Provide ongoing case management support to assess progress and ensure treatment plan outcomes are met or adjusted.
  • Provide field-based therapeutic supportive case management services addressing mental health, functional, and

substance use barriers as needed to help clients obtain and maintain housing.

  • Consult with LCSW and supervisor to conduct crisis and suicide risk assessment and provide appropriate interventions, complete critical incident reporting as required per contract and PATH policies.
  • Provide crisis intervention services focused on enhancing the clients’ ability to independently problem solve, utilize effective coping skills, and manage their own care

WHAT YOU BRING

We’re looking for candidates that :

  • Maintain and execute confidential information according to HIPAA standards
  • Have HMIS experience (preferred)
  • Demonstrate knowledge or experience with case management techniques including critical time intervention, harm reduction strategies, crisis intervention techniques and motivational interviewing
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment

MINIMUM QUALIFICATIONS

To be considered for this position, we require all candidates have :

  • Master’s degree in social work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences or approved by program funder (e.g., MSW, MFT)
  • Current and active licensure or registration with the California Board of Behavioral Sciences (e.g., ACSW, LCSW
  • One (1) years of experience in healthcare, case management, homeless services or working with vulnerable populations.

MANDATORY REQUIREMENTS

Regardless of qualifications, for this position all prospective employees must :

  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR / First Aid training
  • Provide proof of full COVID- vaccination
  • Successfully complete the following as a condition of hire : Tuberculosis TestBackground ScreeningDrug Test

DRIVING

Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must :

  • Have reliable transportation
  • A valid driver’s license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage

FLEXIBLE WORK

We are a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office, does not establish approval for work to be conducted outside of California.
