In this role, you will coordinate long-term care needs for patients within their communities, working in the field and ensuring the best possible outcomes. You will have the support of an elite and dynamic team at your side, all while leveraging your diverse knowledge and experience to enhance healthcare services.
As a Health and Social Services Coordinator, you will play an essential role in our Integrated Care Model, collecting key information regarding member needs, advocating for optimal care, and ensuring access to the right services.
When located in Oahu, HI, you will have the flexibility to work remotely while addressing challenging issues. Expect to spend about 75% of your time visiting our members in their homes or long-term care facilities. Our teams are based in the downtown Honolulu area as well as the West Side, East Side, and North Shore regions. Flexibility, adaptability, and patience are paramount in this role.

Pay is based on various factors, including local labor markets, education, work experience, and certifications.
